The first sets of questions ask you to rate items based on PERSONAL IMPORTANCE. Please identify how important each of these items are to you personally.

The next set of questions will ask you to rate the CONGREGATION’S EFFECTIVENESS. Please rate your congregation on the level of effectiveness for each statement.

Providing your email and first name will allow you to receive a copy of your results. During the GAP Conversations with your church, you will need a copy of your responses.
